The petition of John Blaha respectfully shows:
First. My full name is John Blaha
Second. My place of residence is of Union, Vernon Co. of Wisconsin
Third. My occupation is Farmer
Fourth. I was born on the 31 day of Dec anno Domini 1871 at Hlima Treborn Bohemia
Fifth. I emigrated to the United States from Bremen, Germany on or about the 15th day of June anno Domini 1893 and arrived at the port of Baltimore in the United States on the vessel Weimark
Sixth. I declared my intention to become a citizen of the United States on the 8th day of January, anno Domini 190?
Seventh. I am married. My wife's name is Anna. She was born
and the name, ? and place of birth, and place of residence of each of said children is as follows:

Mary, June 4, 1897, Rosa, May 21, 1899 Anna, April 9, 1905, John, Dec 11, 19??
William Feb 15, 1910 all being born and now reside in Vernon CoWis
Eighth. I am not a disbeliever in or opposed to organized government or a member of or affiliated with any organization or body of persons teaching disbelief in organi? government. I am not a polygamist nor a believer in the practice of polygamy. I am attached to the principles of the Constitution of the United States, and it is my intention ???? a citizen in the United States.
Ninth. I am able to speak the English language.
Tenth. I have resided continuously in the United States of America for a term of five years at least immediately preceeding the date of this petition, in wii, si
