John William Slayback, 81, of Walton, died May 9, 2005, at his daughter's home in Walton. He was a retired truck driver for IMI Concrete Material and a member of Beaver Lick Baptist Church in Union.
He is survived by a daughter, Sheryl Maynard of Walton; two sons, J.W. Slayback of Union and Thomas Slayback of Owenton; two sisters, Mary Frances Conley of Burlington and Betty Knipfer of Ludlow; two brothers, Jim Slayback and Virgil Slayback, both of Burlington; and five grandchildren. He was preceded in death by his parents, John and Lavinia Slayback, and his wife, Ann Slayback.
Visitation will be held from 5 to 9 p.m. May 13 at Stith Funeral Home in Florence. Funeral services will be held at 10 a.m. May 14. Burial will be at Big Bone Baptist Cemetery in Union.
Memorial contributions may be made to Beaver Lick Baptist Church, 11460 Hwy. 42, Union 41091.
