Born to Jacob and Hannah (Unknown) Ellsworth about 1771, Augusta County, Virginia (in the area now known as Germany Valley in Pendleton County, West Virginia). He died between 1828 and 1830 in Madison County, Indiana. He was married in Harrison County, VA, (now WV) on June 28,1792 by the Reverend Joseph Cheuvront, Sr. to Susanna Bumgardner, daughter of Godfrey and Gertrude (Courtrought) Bumgardner. They were parents of ten known children: Sarah, David, Andrew, Adam, William, Hannah, Lydia, Simon, Jacob, and Daniel.

In 1802, John was made one of the trustees for the grounds and cemetery where The Ellsworth Meeting House (later to be known as Bethel M.E. Church) was being built in the Good Hope community in Union Township, Harrison County, VA (now WV). John's uncle, John Ellsworth and wife, Mary (Richards) Ellsworth, had donated the land for the church and cemetery.

In 1806, he migrated to Champaign Co., Ohio (in 1818 the area where he lived became part of Clark Co.), where he is found in the Clark Co. 1820 census. By 1821, the family is in Madison Co., Indiana, where the marriage of their daughter Hannah to Stephen Corwin took place on June 6,1821. This was the first marriage in the county, not yet organized.

John and Susanna owned land and lived in Fall Creek Township where later it became Adams Township. The first marriage in Adams Township was that of their son Adam to Sarah (Sally) Rector in 1825.

The names of John and Susanna Ellsworth are on land transactions through 1828. In 1830 census for Madison Co., IN, Susanna Ellsworth is listed as head of the household, indicating that John had died between 1828 and 1830.

It is believed John and his wife were buried on the home place near Ovid (New Columbus) in Adams Township, Madison County, Indiana, and their bodies have been destroyed. There were reports of gravestones being on the farm years ago, but they have disappeared.
