William Forrest Campbell was the son of Oscar F. and Lillie Boyd Campbell. His maternal grandparents were Harrison Jasper and Mary Caroline Barker Boyd from Tecumseh, Oklahoma. His paternal grandparents were John Allen and Margaret Elizabeth "Bette" Campbell from Harmony, Arkansas.

When Forrest was about three years old, his mother, Lillie, died. He was then raised by his father and both sets of grandparents.

In 1912, at age 12, Forrest was accidentally killed in Shawnee, Oklahoma. He stepped off the curb in front of an oncoming streetcar before his father was able to stop him.

Forrest's funeral was held at the First Presbyterian Church on November 28, 1912. He was laid to rest in the New Hope Cemetery in Tecumseh, Oklahoma.
