Birth: Dec. 31, 1552
Wiltshire, England
Death: Sep., 1611
Greater London, England

Occultist, astrologist and herbalist. Born in Wiltshire, he attended Magdalen College, Oxford. After a short time working as a teacher, he developed an interest in the occult and moved to London. He set up a medical practice in Billingsgate, providing astrologically based remedies. The Royal College Of Surgeons were not happy and banned him from performing surgery, and Forman was jailed soon after following the demise of one of his patients. Following his release he continued disputing the RCS, and managed to gain a licence to practice from the University of Cambridge. His written experiments are now in the care of the Bodleian Library. Shortly after his death, Forman was implicated in the Thomas Overbury murder, as its instigators - Lady Frances Howard and Anne Turner - were among his patients and procured poisons from him.
