The year of Ellsworth's birth is in question, because I've found 4 different years listed. His WWI draft registration shows 1885, which was reported by himself, and the year of 1887 was reported on the 1900 census. Not knowing who reported the information in 1900, I've chosen to use the birth year as 1885 as his year of birth. The year 1884 is on his gravemarker, and the death certificate shows 1886. The informant for the death certificate was his wife, Florence. The month and day of birth have been August 18th, on the records I've found.

"APPLEGATE--Friends of the late Ellsworth Applegate are invited to attend funeral services today (Monday) at 1 P. M. at the Conservatory Chapel of A. J. Rose & Son, E. 6th and Alder."
Both the Oregon death index, and the death certificate, #374, show Ellsworth's spouse as Florence Applegate. The census records, of 1920 and 1930, show she was born in Colorado.
Ellsworth's name has also been found as Elsworth. In the census records of 1920 and 1930, he was listed as Edward, with wife Florence, first in Eugene, Lane County, Oregon and then in 1930, in Liberty Township, Turnbull County, Ohio.
In 1900, Ellsworth was living with his parents, James and Martha Applegate, in Rantoul Township, Champaign County, Illinois. His siblings were listed as Eva, Maude E., Roscoe T., Flossie E., Olive, and George A. In the census for 1910, Champaign Township, Champaign County Illinois, Ellsworth isn't listed with his parents and siblings. His brother George was listed as Arthur, there are two new brothers, Fletcher, and Lewis, and a new sister, Mildred. The Oregon Death Certificate for Ellsworth lists his parents as James Applegate and his mother as Martha, maiden name Taylor.
In 1920, Ellsworth was shown as Edward. He and Florence had a son named James. He was listed as age 11 and born in California. The family were living in Eugene, Lane County, Oregon.
Most of his life, Ellsworth worked as a mechanic. In 1920 he was listed as Foreman, and in other records as a Master Mechanic.
From census records, he lived in Illinois, California, Oregon and Ohio.
