He took off for an anti-submarine patrol in plane # 63915 of VB-105 Squadron on Oct 23,1943, but the Liberator exploded killing all on board. He is also commemorated on the plaque in St. Nicholas Parish church at Dunkeswell Village, UK. "Lest we forget."
