Dr. Gary Joseph Collins, M.D., 42 of Middletown, MD passed away on the evening of September 21, 2008 at home surrounded by family after a brief battle with cancer.

He was born in Salem, OH, December 9, 1965, the son of Bernard and Barbara Collins.

He graduated from Salem High School, class of 1984, Salem, OH. He received a BS in Physical Therapy from Bowling Green State Univresity in 1988 and his medical degree from the Medical College of Ohio in 1994. He completed his intership and residency at Walter Reed Army Medical Center. In 2004, he completed a fellowship in pain medicine at UC Davis.

He served as a physician in the US Army for 12 years, separating as a Lieutenant Colonel in 2007. During this time he was stationed with his family in such locations as Hawaii, South Korea, California, and Washington DC. He was a partner at Parkway Neuroscience & Spine Institute in Hagerstown, Md, practicing as an interventional pain specialist.

Survivors include his loving wife, whom he married July 1, 1989, two sons, two daughters, his parents, a sister and brother Patrick (Amelia) Collins of Dayton, OH, father and mother-in-law, sister-in-law, MD, brother-in-laws four nephews and five nieces, and many other treasured family and friends. He is preceded in death by his sister Mary who died at birth in 1960.

The family will receive friends in Middletown on Thursday, September 25th from 6:00-9:00 pm at the Donald B. Thompson Funeral Home, 31 East Main Street, Middletown, MD.

A memorial mass was held in Salem, OH at St. Paul's Catholic Church. In lieu of flowers, memorial donations may be made to the American Cancer Society at www.cancer.org - please designate "cancer research".
