Hazel Mae Ballard, 87, of Collins, died Thursday in Shell Rock, where she had been living the past four years, from a heart ailment. Funeral services will be 1:30 p.m. Tuesday at the Collins United Methodist Church with Dr. Bob Robinson and the Rev. Paul Frazier officiating. Burial will be in the Collins Evergreen Cemetery.

Hazel Mae Ballard was born Oct. 2, 1913, in Maynard, Minn., the daughter of John Thomas and Katie (Kaltenhauser) Holland. She married Claude William Ballard on Feb. 2, 1935, in Des Moines. He died on Aug. 3, 1969. She was a homemaker and had also worked for 18 years as a seamstress for Willard's Ready-to-Wear in Marshalltown. She was a member of the Collins United Methodist Church, where she was former president of the UMW, the Kinzington Unit, and the Collins garden club. She is survived by two sons, the Rev. William Ballard of Eagle Grove and George Ballard of Mitchellville; one daughter, Jane Kliebenstein of Shell Rock; six grandchildren; 11 great-grandchildren; and one sister, Mabel Justice of Bondurant.

She was preceded in death by her husband and two brothers.

Visitation begins at 4 p.m. Monday at the Collins United Methodist Church with the family present from 5 to 8 p.m. A prayer service will be held at the church at 7:45 p.m. Memorials can be made to the Collins United Methodist Church and the Iowa Commission for the Blind. The Bacon Funeral Home in Maxwell is in charge of arrangements.

Survivors include two sons, the Rev. William (and Sue Ellen) Ballard of Eagle Grove, and George (and Kristy) Ballard of Mitchellville; a daughter, Jane (Glen) Kliebenstein of Shell Rock; six grandchildren; 11 great-grandchildren; and a sister, Mabel Justice of Bondurant.

Her parents, husband and two brothers, Everett and Claude, preceded her in death.

Funeral services were Tuesday, June 12, at the Collins United Methodist Church with Dr. Bob Robinson and the Rev. Paul Frazier officiating. Burial was held in the Collins Evergreen Cemetery.

Memorials may be made to the Collins United Methodist Church and/or the Iowa Commission for the Blind. The Bacon Funeral Home in Maxwell handled the arrangements.
