Dale Jacqueline Perez died on March 28, 2012 as a result of complications of diabetes and kidney disease. Although the diabetes blinded her, she did not allow that to interfere with her love for her partner of 15 years or from encouraging others, including her daughter, to live a healthy lifestyle to keep diabetes from occurring.
At Dale's Memorial Service held April 10, 2012, her daughter shared that with her mother's encouragement, she has broken the family pattern of diabetes due to better health habits and lifestyle. She stated that if thirty years ago people knew what is known now, it would have saved countless lives.
Louie Ramos was Ms. Perez's case manager for eight years. During that period, she went blind. "She had multiple health issues, including asthma," Ramos said. "But she never cared about medical issues— she was always high-spirited. And serving her in the hospital, I saw she was an inspiration to others." "Her love for her partner and her pet were her beauty," said a woman who knew her. "She never complained."
Dale Perez was proud of her daughter who is finishing a degree at SFSU. In Dale's final days, her daughrer surrounded her with the Motown music she loved and she left this Earth to the sounds of one of her favorites, "Ain't No Mountain High Enough."

A search of ancestry.com returned the SSDI which gives her birth date. Public records show she lived in Daly City, CA in 1986 and in 1995 and after, she lived in San Francisco. SF Mom
Cremated, Ashes scattered at sea.
