482859 Private Thomas Cooper 1st Battalion Royal New Zealand Infantry Regiment attached to 4th ANZAC Battalion Royal Australian Regiment.
On 10 October 1970 3 Platoon [3Pl] suffered a 'blue on blue' [friendly] accident when one element fired on another element by mistake. Friendly fire killed Pte Tom Cooper, crippled Pte Ian Herd and wounded Lcpl 'Sammy' Samson and Pte Ross Dunlea. The accident cast a pall over the otherwise successful W3 tour and left the company personnel traumatised, saddened and frustrated.
Died of Wounds - gunshot wound to right scalp sustained in a friendly clash at 24th US Evacuation Hospital Vietnam 11th October 1970 Age 22.
Private Cooper's brother, Mr Martin Cooper of Auckland, received the New Zealand Operational Service Medal on his behalf from the Prime Minister and the Minister of Defence at a presentation at the Auckland War Memorial Museum on Thursday 27th November 2003.
