Hazel Bernice Cook, 95, a resident of Northbrook Manor Care Center, formerly of Central City, died Saturday, Dec. 23, 2006, in the care center following a long illness.
Hazel was born, the daughter of Fred and Lizzie (Thompson) Schultz, on June 25, 1911, in Marion. On March 14, 1932, she was married to Herman Hackert in Cedar Rapids. He died May 3, 1973. She then married Robert Cook on Jan. 11, 1975, in Hiawatha. He died Nov. 3, 1977.

Hazel was a member of United Church of Christ in Central City, Agoma Class, Friendly Garden Club and Northeast Linn Senior Citizens Club. For several years Hazel resided at Four Oaks in Central City.
