Age 74, daughter of the late Peter and Agnes Gajos Kowalik. She was a member of St. Ambrose Catholic Church, where she volunteered in many church groups including St. Ambrose Women's Guild. She was a member of the AARP, and was an elections worker for Medina County. She enjoyed gardening, bowling and traveling.

She was preceded in death by her husband of 36 years, Raymond S. Cieslak; and two brothers, Matthew and Theodore Kowalik.

She is survived by two sons, Lt. Col. Dr. Theodore Cieslak of Woodsboro, Md., and his wife, Barbara, and Dr. Paul Cieslak of Portland, Ore., and his wife, Christine; two daughters, Kathleen Cieslak and Diane Cieslak, both of Brunswick, Ohio; a sister, MaryJohnson of Erie; and six grandchildren.
