Mary died at McCormick Hospital in Moberly after a 31 day hospitalization. She had been living in Moberly prior to her hospitalization.
She was a widow ( a widow twice)
According to her death certificate,
she was buried in Madison, MO but a cemetery was not listed.
So if anyone sees her grave marker in this cemetery, please let me know and take a photo if possible. (See July 2013 addition below.)

She was married to Garrett H Richardson on Feb 28, 1888 and as supported in the 1900 and 1910 census records. They divorced before 1913. He died in 1922 and was divorced at that time.
On 18 Mar 1913, she married Jason M Nichols and was listed as Mary Richardson.
In the 1930 census Mary E was living with Jason M Nichols in Randolph Co, MO
In 1910 Garrett and Mary Richardson were living with Jason Nichols.
Mary Nichols was the informant on Garrett Richardson's death certificate.

From Moberly Monitor-Index, Wednesday, 12-8-1948,
MRS. MARY NICHOLS DIES; FUNERAL TO BE HELD TOMORROW
Mrs. Mary E. Nichols, 84, died at 12:50 o'clock yesterday afternoon in McCormick Hospital. She had been ill since sustaining a fractured hip November 6.
Mrs. Nichols, the widow of Jason M. Nichols, who died in November, 1936, leaves no survivors. She was born in Fayette and had lived in Moberly 30 years.
The body is in the Mahan Funeral Parlors, where services will be conducted at 10:30 o'clock tomorrow morning by the Rev. Roy Heifner of Moberly, Baptist minister. Burial will be in Oakland Cemetery.
