Louis Kovachi, miner, killed by fall of rock in mine No. 8, Sheridan Coal Co., located at Breezy Hill, on March 7, 1913. This was the first fatal accident to occur after we assumed the duties of this office. Deputy Inspector Clark made the investigation and found that a rock measuring 13 feet long, 6 feet wide, and about 5 inches thick, had fallen on Mr. Kovachi at his working face while he had been in the act of mining off a shot. Mr. Clark took statement from a number of witnesses.
