Member of 50 Year club, Illinois State Medical society and Surgical staff of Roseland Community Hospital, and for 40 years surgeon for the Nickel Plate R. R.

In lieu of flowers, please make contribution to the Cancer fund. Interment St. Nicholas cemetery. BR 8-7767.

Dr. Siemens was a veteran of WW1

Dr.Siemens was one of the lead founders to bring the Ukrainian Pavilion to the 1934 Worlds Fair to Chicago.
